HIB Reporting Forms

REPORTING BY FAMILIES/CAREGIVERS

 

The form linked below should be used by parents or guardians to report allegations of HIB. The form can be filled out online or printed and filled out by hand. Once completed, the form should be forwarded to the School Anti-Bullying Specialist electronically or turned into the main office to the attention of the School Anti-Bullying Specialist. An investigation will be completed no later than 10 school days from the date the form is received. Should you have any questions, please contact the assigned person. 

 

REPORTING BY SCHOOL DISTRICT PERSONNEL AND CONTRACTED SERVICE PROVIDERS

 

The form linked below is for school district personnel or contracted service providers only. Such individuals are required to submit a written report to the principal or School Anti-Bullying Specialist within two days of witnessing or receiving information that a student has been subject to HIB. Once completed, the school district personnel must promptly submit the form to the school principal or main office.
